Photography, Videography, and Media Release Consent Form 

By signing this form, you grant Food Fleet and its authorized representatives permission to photograph, record, and use your image, likeness, voice, and related media content for marketing, promotional, and commercial purposes as described below. 

Grant of Rights

I hereby grant to Food Fleet, its affiliates, agents, licensees, successors, and assigns (collectively, "Food Fleet") the irrevocable, royalty-free, worldwide, and perpetual right and license to: 

  • Photograph, film, record, and capture my image, likeness, voice, name, and performance in any media format. 

  • Use, reproduce, publish, distribute, display, broadcast, edit, modify, and create derivative works from such content. 

  • Use such content for any marketing, advertising, promotional, commercial, editorial, or educational purpose, including but not limited to social media, websites, print materials, video content, email campaigns, trade show displays, and press materials. 

  • Use my name, image, or statement as a testimonial or endorsement in connection with Food Fleet’s brand, products, and services. 

Terms & Conditions

Duration & Perpetuity: This consent is granted in perpetuity and does not expire. Food Fleet may continue to use captured content indefinitely, even after the termination of any business relationship. 

No Compensation: I understand and agree that I will not receive monetary compensation or royalties of any kind for the use of my image, likeness, or voice. 

No Approval Rights: I waive any right to review, approve, or object to the final form of any content in which my image, likeness, or voice appears. 

Release of Claims: I release Food Fleet, its officers, employees, contractors, and agents from any claims, demands, or causes of action arising from the use of my image, likeness, or voice as described herein, including claims related to defamation, invasion of privacy, right of publicity, or copyright infringement. 

Governing Law: This agreement shall be governed by and construed in accordance with the laws of the State of California, without regard to conflict of law principles. 

 

Minor Consent (If Subject is Under 18) 

If the subject is a minor (under 18 years of age), a parent or legal guardian must sign this form on their behalf. 

Acknowledgment & Signature 

By signing above, I confirm that I have read and fully understand this Photography & Media Release Consent Form, that I am 18 years of age or older (or am the parent/guardian of the minor named above), and that I agree to all of the terms herein voluntarily and without duress.
